I [Charles Cardess] do hereby do hereby desire
you to promise for me an Annuity Note of | Here give the

with that of
the person's condition
in life

value six guineas £6:6s7 principal money
so soon as the same can be obtained from
his Majesty's Principal Annuity Note Office
at Westminster in exchange for the Annuity
Note No [4775] by reason that the same
is become so worn or torn as by wear and
tear reduced to such a state as to be differenthas become unfit<add>unfit</add>
to circulationfor circulation to which intent I do hereby deliver
to you the said Note No 4775. in order that the
same may be transmitted to thehis Majesty's Principal Annuity
Note Office in Westminster , to be examined
and exchanged in the said Office. —

delivered being in a state unfit for general circulation,
I hereby desire you to deliver to me
Your Receipt for this same according to the Form
by law appointed, and upon the re-delivery ofreceiving back
