Metasylum Man]</p><p>Place & Date</p><p>Be it remembered that on this day came before<lb/>me A. J. one of his Majesty's Justices of the Peace for<lb/>__________ came F. G. of [<add>Place</add>] [<add>Occupation</add>] and<lb/>declared as follows, viz:</p><p>1. That there is now under confinement in the General<lb/>Panopticon Penitentiary House as Penitentiary-Man<lb/>one A. P.: and that the term of the aid A. P. will<lb/>expire on the ____ of <add>Month</add> next. [Or if in the<lb/>Metasylum or serving according to any of the other<lb/>options, state accordingly.]</p> | |||

Be it remembered that on this day came before
me A. J. one of his Majesty's Justices of the Peace for
__________ came F. G. of [Place] [Occupation] and
declared as follows, viz:
