Art. 2. Heads under what in the Death-recording
Register entries will be made are the following: adding in
each instance one or another of the three words following to wit — known, supposed, or unin

I. Place designated
in the most particular
manner as per Election

2. Names at length as reported exceptions excepted and whether as known or as supposed.

most particular manner as per Election Code §.

III. Time of death, to wit Year Month, day of the month.

IV. 1. Cause of death — and whether mere old age, illness, or
casualty. and whether known, conjectured or .

2. If illness disease name of the disease, and whether as known or
supposed

3. If casualty, 1. what; and whether any and what person and whether as known or as supposed<lb/> or person is or were known or were supposed to have contributed,
2. Persons contributing to it any and when.
