Recitals
1. Whereas by the List of Penitentiary Admissions received
from you by this Court on the day of [Month Year] it appears that
a Felon Convict whose Identification-Mark (Letter for Letter and Figure
for Figure) is [Q.P. &c. ] was on the day
of [Month Year] admitted into your said House.
2. And Whereas by the List of Penitentiary Discharges received
from you as aforesaid on the day of [Month Year] discharged
from thence [or in case of escape say did on the day &c escape from thence]
and that therefore he the said Q.P. had in quality of a Penitentiary Prisoner
under the said Act continued under your care for the full term of four years:
according to which term the sum of is in your said List stated as the maximum
which on account of any Post-Liberation Offence or Offences committed by the said
Q.P. you can be required to pay.
3. And Whereas a Prisoner who by his identification mark appeared
manifestly to be the said Q.P. hath now on this day of [Month Year]
