Certificate of Discharge & Inlistment under the
Statute 34. Geo: 3. ch. _____ § VII attesting the discharge of
A. L. from the General Panopticon Penitentiary
House, and his Inlistment into the service of his Majesty
[or of the East India Company.]

General Panopticon Penitentiary House this _____ day of
[Month Year]

On the _____ day of [Month Year] A.D. the intended
Bearer of this was discharged from the General Panopticon
Penitentiary House or from the Metasylum or
from naming whatever other service he had been last engaged
in.

On the same day he was inlisted in the service of his
Majesty in the [Corps] or on board the [Ship]
or the East India Company as the same may be.

This Certificate is accordingly given him for his
protection that he may not be deemed as a fugitive
from the Penitentiary House or a deserter from his
service due in virtue of any subsequent engagement.
