§ 2

And be it further enacted that the said
Commissioners shall moreover frame or cause to
be framed a set of blank forms or schemes
exhibiting under proper heads a memorandum the particulars an account of the commitment
receipt of each person who shall have been
committed to any Jail or other place of lawful
confinement within this United Kingdom (including
such places as are or shall be used for the confinement
of persons insane) together with the cause and
other proper particulars relative to such commitment:
as also another set of blank forms or schemes
exhibiting a memorandum of the discharge departure of
any such person, whether by death or otherwise escape, discharge or otherwise, from every such commitment
as also another set of exhibiting a list Register or Calendar of
all persons so received and discharged departed in the course
of a certain period of each Yearly, quarterly, monthly or other proper and convenient period
to be appoin from
time to time appointed
by the said Commissioners
and of all persons remaining
at the end thereof from in such Jail or other
place of lawful confinement And such blank the said Commissioners

forms, memorandums of commitment receipt and Discharge
departure respectively shall by the next post
after any each such receipt or departure
after such disch commitment or discharge
shall transmitt a proper and sufficient number
by post or otherwise to every Keeper of any Jail or other
such lawful plan of confinement within this United Kingdom
a proper and sufficient number of each such set
of blank schemes forms or schemes together with proper and
sufficient Instructions for the filling up of the same respectively; And
so soon as any such set of blank Forms or Schemes shall
have been <add>so received by any such Keeper as aforesaid, every
such Keeper by whom the same shall shall have taken place shall so have been
received shall after
each such receipt
or departure of any person as aforesaid transmitt to the said Board
a proper and
sufficient memorandum
thereof,
expressed by the
filling up of
some such blank
form or scheme
as aforesaid or
otherwise, shall
by the next post
be returned or transmitted by
every such Jailor or Keeper of such place of
lawful confinement to the said Board: and
every such periodical Register or Calendar shall be returned or transmitted
