Art. 1. The purpose thus the Judge proceeds Art. 1. When The purpose of the application being has or if he reason

by himself or the Registrar as the case may be
Col. I Year, month, day and time
Col. II. As p. expressive of the place of the Applicant in the series
of that day's Applicants.
Col. III. Applicants name at length
Col. IV Applicant Age, Year Month and day of birth so far as known
Col. V. Applicants condition in life, in respect of marriage.
Batchelor, Married then Widower. Spinster, Married
Woman, Widow.
Col. VI. Applicants abode habitation expressed as per Election Code §
Col.; VII Applicants occupation
Col. VII Means of intercourse with Applicant for a time be
at first limited, and from time to time renewable: abode
at which any thing addressed to him being delivered shall
be promised to have been received by him

Produce your Applicant's Address paper ready filled up,
or make answer to such questions as I shall have to
put to you for the purpose of filling up this which I have
in my hand.

Art. 2. If by the applicant a paper ready filled
up is produced accordingly the Judge either, by the word allowed with the addition of his
signature
signifies his
satisfaction with it as it stands, or proceeds and continues to
put the appropriate questions, untill being brought to an appropriate
state it receives his allowance, as above. For an Applicants address paper see the
