Superannuation Annuity

yearly payment, and praying that the same may be remitted to him through your hands,
And Whereas the amount thereof being [£ S d] was accordingly [this] day paid in
on my account to the Post Office in London as appears by a Receipt in my possession
signed [Name & Stile of office] This is to desire you in pursuance of the Statute
of the [34. Geo:3.ch II 6 art. ] to pay over to the said R. A. accordingly
the said sum of [£ S d] in one payment on the day of [Month Year] or so
soon after as he shall make application for the same deducting the sum of [1S] as
your fee. [or in monthly instalments commencing on the day of [Month Year] deducting
each time the sum of [3.d] as your fee][or in weekly payments commencing
the day of [Month Year] deducting each time the sum of [1d] as your fee; as
witness my hand
