Accreditation Certificate in favour of a Friendly Guardian
This is to certify whom it may concern, that J.A.M. [Rector Vicar or Curate] of
the Parish of... aforesaid being the Parish in which the above named F G
is a Housekeeper as aforesaid having considered the above examination, and do believe the
same to be true; and having for [Years or Months] past known the said Examinant, do
believe him to be a person of unexceptionable character, fit as such to be accepted in quality of
a Friendly Guardian under the Statute aforesaid. Witness my hand this... day of [Month
Year]

a full direction, adding thereto
the following words, by which
direction Letters will reach one by
the Post.
NB. If the person granting this Certificate be a Church warden, he must, in addition
to his name, mention his occupation, and state whether it is for the existing year that
he is Churchwarden or for any and what preceding year he was so.
NB. This certificate must, if not signed by the Minister, be signed by two persons who
either are or have been Churchwardens for some Parish in which the Examinant is or has
been resident.
If signed by a Justice of the Peace, it may be by the examining Magistrate or
any other Justice of the Peace for any County or Town in Great Britain: but he must
shew for what place, and shew how he may be directed to, as above.
