to the Justices of the Peace for the County of [ ] in General Sessions, praying
in virtue of the Statute 34. Geo: ch. an Order of Deputation to a Justice for taking the Subsidiary
Examination of A.E. touching the place of Settlement of A.P. Penitentiary Man
in the General Panopticon Penitentiary House.

Sessions assembled.
The Petition of A.O. B.O. &c Overseers of the Poor for the Parish aforesaid humbly sheweth
1 That on the day of [Month Year] your Petitioners received a Paper purporting to be
Recital: viz:
1. Receipt of Order of Adjudication
an Order of Adjudication from A.J. & B.J. two of his Majesty's Justices of the Peace for the
County of acting in virtue of the said Statute § under a Deputation from the
Justices of the Peace for the said County in General Sessions assembled, and declaring the last legal
Settlement of the said A.P. to be in our said Parish, together with the Examination of the said
A.P. (Here also mention the Examinations, if any, of any other witnesses) being the evidence
on which the said Order was grounded.
2. That for the better information of your Worships in this behalf we have to this our Petition
annexed a true Copy of the Order of Adjudication aforesaid marked [B] as also a Copy of the
Examination aforesaid marked [C.]
3. That notwithstanding that in consequence of the said Examination it is by the said Order adjudged
and declared that our said Parish is the place of the last Legal Settlement of the said A.P. we
do in our consciences believe [or suspect] that his said Settlement is not in our Parish: inasmuch as
in respect of the matters of fact supposed by the said Adjudication there is material error: viz: [Here
state the error or errors suspected]
4. That we understand [or are induced to believe] that A.W. [or that a person who by the best
description we can as yet obtain of him appears to be A.W.] of [Street Town] or [Parish] as the case
is] in your said County could if duly examined give material information touching the premises: inasmuch
as [Here state what the supposed Witness is supposed to know relative to the business]

Prayer: viz: for
Wherefore in pursuance of the said Statute our humble Petition as as follows: viz:
1. Order of Adjudication.
1. That your Worships would be pleased with all convenient dispatch to make an Order of Deputation
deputing some one of yourselves or your fellows, to take the Examination of the said A.W.
touching premises (according to the Instruction. hereunto annexed marked E) as also of any other
persons whose evidence may to such Magistrate appear likely to be material thereto and capable of
being obtained: for which Order of Deputation we herein also inclose a blank Form marked [D]
with which for that purpose we have been furnished by the Governor aforesaid.

2. That, together with such Order of Deputation, properly filled up your Worships would also be
pleased to cause to be delivered to such Magistrate as well the said Order of Adjudication & Examination
as the said annexed Paper of Instruction, directing him to have due regard thereto, so far forth as
the same may be proper and by Law fit to be observed.
