of Superannuation-Annuity money.

Whereas on the day of [Month Year] I received
a Petition for Remittance signed R. A.
with your identity certificate in favour of the said R.
A. annexed representing to me that on the day
of [] next ensuing there [will have] become
due to him [£ s d] upon his Superannuation Annuity
[one] half yearly payment, and praying that
the same may be remitted to him through your
hands. And Whereas the amount thereof being
[£ s d] was accordingly [this] day paid in on
my amount to the Post Office in London as appears
by a receipt in my possession signed [Name and
Stile of Office] This is to desire you in pursuance
of the Statute of the [34. Geo: 3. ch. §. 6. art.]
to pay over to the said R. A. accordingly, the said

[Month Year] or so soon after as he shall make
