This is to certify whom it may concern, that J.
A. M [Rector Vicar or Curate] of the Parish of
aforesaid being the Parish in which the abovenamed
F. G. is a Housekeeper as aforesaid have considered
the above examination and do believe the same to be
true: and having for [Years or Months] past known
the said Examinant, do believe him to be a person
of unexceptionable character, and fit as such to be
accepted in quality of a Friendly Guardian under the
Statute aforesaid. Witness my hand this _____ day
of [Month Year]

direction, adding thereto the
following words: by which
direction Letters will reach me by
the Post.
N. B. If the person granting this certificate be a
Churchwarden, he must, in addition to his name,
mention his occupation, and state whether it is for the
existing Year that he is Churchwarden or for any
and what preceding year that he was so.
N. B. this certificate must if not signed by the
Minister, be signed by two persons who either are or
have been Churchwardens for some Parish in which
the Examinant is or has been resident.
