Certificate of Reformation and Good-Behaviour
from the Governor of the General Panopticon Penitentiary
House, in favour of A.P. quondam Penitentiary-Man
therein; under the General Panopticon
Penitentiary Act 34 Geo: 3. Ch. §.VII.
General Panopticon Penitentiary House, this day of [Month Year]
This is to certify whom it may concern —
1. That on the day of [Month Year] the said A.P.
was committed to this Establishment.
2. That now on this present day he is charged therefrom.
3. That under the discipline thereof his behaviour
is become diligent, orderly, and unexceptionable.
4. That he has testified declared his sorrows for his past
offences and his resolution henceforward to lead
the life of a good and honest member of society.
5. That in such his declaration I do to the best
of my judgment believe him to be sincere. Witness my hand

In the case of an engagement in either branch
of his Majesty's Service may be added as follows —
6. That being ambitious of being reserved into engaged in his
Majesty's service by Land in the [Corps] or
by Sea on board the [Ship] should he be thought
fit to worthy the terms of being received into the
Company
