Accreditation Certificate in favour of a
Friendly Guardian
This is to certify whom it may concern, that
I. A. M. [ Rector Vicar or Curate ] of the Parish
of aforesaid being the Parish in which
the abovementioned F. G. is a Housekeeper as aforesaid
have considered the above examination, and do
verily believe the same to be true: and having
for [ Years or Months ] past known the said Examinant,
do believe him to be a person of unexceptionable
character, and fit as such to be accepted as in quality of a Friendly
Guardian under the Statute aforesaid. Witness
my hand this day of [ Month Year ]
A. M. Here give a
full direction, adding thereunto
the following words by which
direction Letters will reach me
by the Post.
N.B. If the person granting this certificate be a Churchwarden,
he must, in addition to his name, mention his occupation,
and state whether it is for the existing Year
that he is Churchwarden or for any and what preceding
Year that he was so.
N.B. This certificate must, if not signed by the Minister,
must be signed by two persons who either are or have
been Churchwardens for some Parish in which the Examinant
is or has been resident.
If signed by a Justice of the Peace, it may be by the
Examining Magistrate or any other Justice of the Peace for any County or other
Town in Great Britain: but he must show for what Place, and shew how he may be
directed to, as above.
